New Hampshire CNA certification: stay active, lapse, and transfer rules

New Hampshire licenses LNAs (a license, not just a registry listing), renewed every two years by your birthday. Competence at renewal or reinstatement can be shown three ways under Nur 401.05: at least 200 hours using nursing-assistant knowledge and skills in the prior 2 years (the hours may be paid or unpaid, including qualifying family care under a nursing plan of care), OR at least 24 contact hours of continuing education in the prior 2 years, OR completion of a Board-approved nursing assistant program within the past 2 years. The 200-hour route is the standard path; the CE and program routes are real alternatives older guides miss.

Continuing education24 contact hours of continuing education within the two years immediately preceding renewal (conferences, lectures or other offerings designed to enhance nursing assistant knowledge, judgment and skills), logged in CE Broker, which OPLC made the only approved CE tracking system. Successfully passing the nursing assistant competency test within the two years can substitute for the CE/practice evidence in reinstatement/endorsement contexts.

Changing soon: OPLC consolidated LNA fees into Plc 1002.33 effective July 1, 2025 ($38 base + $28 PHP = $66); pre-2025 schedules quoting $25, $35 or $63 are stale. CE Broker became the only approved CE tracker on February 3, 2025.

If your New Hampshire certification has lapsed

The cheaper route — while it lasts

Expired less than one year: you cannot renew online; mail a paper reinstatement application with the fee ($66 under the current Plc 1002.33 schedule: $38 + $28 PHP fee) plus documentation of either 200 hours of supervised nursing-related duties in the past two years or written and clinical competency test results, and proof of 24 CE contact hours or exam results.

Past that line

Expired one year or more: apply again as a new applicant through the online portal, with a new criminal background check. Separately, the federal clock still applies: after 24 consecutive months without paid nursing or nursing-related work, a new competency evaluation is required before a Medicare/Medicaid-certified facility can employ you as a nursing assistant.

Trap: working on an expired LNA license can bring a fine of up to $50 for each month worked expired. Two clocks run at once in NH: the license expiration/birthday clock (paper reinstatement only within 1 year) and the federal 24-month no-work clock.

Transferring INTO New Hampshire

Application fee: $66. Active license in good standing in another state, plus documentation of 200 hours of nursing assistant practice under LPN/RN/APRN supervision within the prior two years and 24 contact hours of continuing education within the prior two years (applicants whose clinical/skills assessment was within the prior two years may be exempt from the practice/CE documentation). Submit license verification, CE audit form, and a NH Department of Safety fingerprint background check ($48.25, separate). Work-history requirement: 200 hours of nursing assistant practice under nurse supervision within the prior two years. The fee is $66 in total: a $38 application/licensing fee (Plc 1002.33, effective July 1, 2025) plus the $28 Professionals' Health Program fee (Plc 1001.08) — the same $66 applies to initial, renewal, reinstatement and endorsement applications. The $63 figure in older guides was $35 + $28 under the pre-July-2025 schedule. A criminal background check fee is extra.

The table below is computed for an active aide in good standing, trained to the origin state's standard hours. Your own hours or status can change your row — run the transfer checker with your real facts.
